"This is particularly disappointing given the consistent improvement the industry has seen over the past two decades," the chamber said.
It said there had been a number of accidents where up to five people had died in recent weeks, related to seismic activity and subsequent fall of ground.
The chamber said fatalities from fall of ground incidents had declined by 88% between 1993-2016 and said the issue was an industry focus for CEOs, particularly at deep mines.
AngloGold Ashanti (SJ:ANG) operates the world's deepest mine, Mponeng, and the company's South Africa COO Chris Sheppard will lead a task team to develop a summary of rockburst leading practices and share the results.
The chamber also recently raised concerns about violence in mining communities, including assassination-style killings and the murder of Harmony Gold's (SJ:HAR) Tshepong and Phakisa regional mine manager in late September.
